Nursing Information System(NIS) ia a part of a health care information system that deals with nursing aspects, particularly the maintanence of the nursing record.
Nursing's integral contribution to the process of selecting and
implementing a hospital information system
is essential to the sucess of the system. This process defined as
the systems life cycle,comprises four
phases:Analysis,design,development and implementation.
For the implementation or upgrade of Nursing Information System a
NIS committee comprised by different health
care proffessionals,is necessary to be formed. The implementation
and upgrade of a Nursing Information System
follows some specific phases.These steps,include
planning phase,analysis phase,design phase,development
phase,implementation phase and upgrade phase.
The Implementation of a mobile Nursing Information Systemhas been shown to be beneficial for nurses ,taking away the burden of paperwork and enabling them to focus more on patient care. However ,the factors that contribute to a sucessful MNIS implementation have been not been widely studied.
The five steps of
Nursing Implementation System are:
a)Assessment
Phase: The first step of the nursing process is
assessment.During this phase the nurse gathers information about a
patient's psychological,physiological,
sociological and spiritual status. This data can be collected in a
variety of ways.
b)Diagnosing
Phase:The diagnosing phase involves a nurse making an
educated judgement about potential or actual health problem wiith a
patient.
Multiple diagnosis are sommetimes made for a sinnglle patient. The
diagnosis phas is a critical step as it is used to determine the
course of treaatmeennt.
c)Planning
Phase:Once a patiient and nurse agree on the diagnosis, a
plan of action can be developed. If multiple diagnosis need to be
addressed ,the head nurse will prioritize each
assessment and devote attention to severe symptooms and high risk
factors. Each problem is assigned a clear, measuable goal for the
expected beneficial outcome.The Nursing Interventions
Classification
may also be used as a resourse for planning.
d)Implementating
Phase:The Implementing phase iis where the nurse follows
through on the decided plan of action.Actions involved in a nursing
care plan include monitoring the patient for signs of change or
improvement, directly caring for the patient or performing
necessary medical tasks, educating and instructing the patient
about further health management, and referring or contacting the
patient for follow-up. Implementation can take place over the
course of hours, days, weeks, or even months.
e)Evaluation
Phase:The possible patient outcomes are generally
described under three terms: patient's condition improved,
patient's condition stabilized, and patient's condition
deteriorated, died, or discharged. In the event the condition of
the patient has shown no improvement, or if the wellness goals were
not met, the nursing process begins again from the first step.
