A. Initially after admission detail history should be included.

First present history of medical illness. It includes present condition of patient, investigation performed medication and other management

Past medical history includes details of prior admissions and details.

Past and present surgical history should include details of surgeries patient undergone in past and present situation.

Collect family history with any hereditary disease and also personal history. Personnel history includes habits, diet pattern, sleep pattern, elimination status etc. And also collect socio economic status.

B. History collection plays an important role in assessing patient condition. Based on the data collection history should be checked for history of diseases. These helps in diagnosis of patient condition.

C. Initially data collection is started with patient if possible otherwise can be collected from primary care givers. And details about previous ous admission, procedures performed etc can be checked from patient medical record. And also through assessment can identify patient problems.

D. Nursing management is required for patients who are hospitalized. Effective nursing care help the patient for speedy recovery. They assessed patient condition and based on the problems planning should be done
