Ruby lives in the community. She is very independent at 89. Ruby has had a hip replacement, and has difficulties walking long distances, but she prefers to walk when she can. She is about a ten minute walk from the shops and often walks into town for a coffee, or to buy her groceries.

Walking home one day, Ruby trips on a broken paver and falls. She cannot get up. A neighbour runs to help and calls an ambulance. Ruby spends four nights in the hospital. She wants to go home, but the health professionals want to make sure Ruby is stable and safe before any decisions are made

1. You are providing support services to Ruby and have identified with her that her goal is to continue living independently in the community. When planning service delivery, what types of resources would you look at in order to deliver identified services that will allow her to do this?

I will help to arrange home based services in a holistic manner. The following type of resources will be identified.

Health aides. Health aides can help Ms. Ruby in her basic personal needs such as getting out of bed, walking, sitting, bathing, eating and dressing.

Nursing and doctor care: Will arrange home based nurse care twice in a month and physician care once in a month to routinely check her health status

Physiotherapy: Will arrange a physiotherapist to help her to regain or strengthen use of muscles and joints.

Pharmaceutical services. Medicine will be delivered at home as per the order of the doctor

Transportation. Will arrange transportation to and fro to a medical facility for treatment or for physical examination or for other purposes

Home-delivered meals. Will offer meal services at her home depends up on her needs and when she became able to cook, will arrange food stuffs to cook.

Home maker: Arrange a person to do household tasks for herself for few months and after that, will give instruction to assist her in her household activities

2. Explain three ways in which you would support Ruby to access and negotiate resources

1) Will identify government schemes which can help her financially and socially. Will talk with concerned people and will do the needful to arrange that

2) Will identify the home based health care service centers (public and private) and will register her for home based health care services

3) Will identify special helping strategies for geriatric population which helps with various services including, Pharmaceutical Assistance Programs, house cleaning, personal care assistance, meal preparation, money management, transportation support etc.

3. What are two actions that should be taken if one of the services Ruby requires is no longer able to provide the level of support required?

First of all I will identify the unmet needs and will try to do the following actions

1. Will discuss with her doctor if it is something related to her health status. identify a proper organization or team who can give her that support through cost effective manner (eg: Palliative care group)

2. Will arrange a volunteer from her own community and will arrange a group with his/her leadership and will arrange for services as per the need. This volunteer can daily monitor her needs and then can help me to gibe services as per her need

4. How could you ensure that the action you take to minimize disruption to service delivery if a service is no longer able to provide the level of support Ruby requires? Identify two methods you would use.

        The methods are the followings

1. I will properly monitor her physical, social, emotional and spiritual health status through proper methods and will identify the unmet needs.

2. Will ensure the services of community volunteers who can identify her needs properly and will measure the quality of life after getting interventions

5. Who could you consult when making decisions about Ruby’s new service delivery plan? approx. 25 words

First of all I will consult her doctor and nurse will identify about the do’s and don’ts needed to ensure while giving services to her. Then after getting proper information, I will consult a palliative care multidisciplinary team including doctor, nurses, social workers, volunteers, physiotherapist, and counselors. Then will seek their help in delivering services to her in a holistic manner
