The possible nursing diagnosis for the initial 2 hours of age of newborn are as listed below ;-

  • The risk for infection related to neonatal immune system ,Even though theh newborn is healthy , but there are still at the risk of certain infection that depending on the people they come in contact with , especially if they remain unvaccinated for the first 4 weeks ( HBV, BCG , and OPV vaccines that are usually given at birth ) and if the umbilical cord stump gets contaminated with the urine or fecal matter due to poor cord care .
  • The risk for impaired skin integrity related to the external conditions ( eg. heat rash - milaria, can also get diaper rash as well. )
  • The risk for imbalanced nutrition that less than body requirements related to expected weight loss . NEwborns lose about 7 - 10 % of their birth weight during the first few days of life ( physiologic weight loss ) .
  • The risk of hypothermia , hypoglycemia and the feeding issues that are related to small for gestational age or large for gestational age infants
  • High risk of impaired gas exchange related to decreased production of surfectant .
  • Ineffective breathing pattern related to prematurity
  • The risk eating disorder related to insufficient sucking and swallowing reflexes
  • potential risk of altered nutrition ;- related to less than body requirement , by raising the metabolic rate
  • Potential risk for fluid volume deficit related to increased insensible water loss
  • The risk of disturbance in the body temperature associated with immaturity .
  • The high risk of tissue damage related to invasive procedures during birth.
  • High risk of activity intolerance and poor oxygenation related to weakness
  • Risk of impaired parenting by the separation of mother and the child .
