Transitional care : it is a coordination and continuity of health care during a movement from one health care settings to another or to shifting to home, and care provided by health care professionals or providers.

Documentation : means maintainance of records and reports after providing care to the clients.

SRMC : means society of risk management consultants.

If hospital does not using this SRMC, nurse responsible for on going care of newborn is :

* continuous monitoring of vital signs,

* maintainance of rooming in, to early recovery of newborn,

* if mother is not available providing formula feeding.

* maintaining adequate warmth ness for new born, by using radiant warmer.

* maintain adequate skin care, to prevent perineal rashes.

* provide comfortable position and maintain good nutritional status and allow mother to learn new born care.

* maintaining documentation.

This is the nurses responsibility for ongoing care of newborn.
