Comunity care should focus on family, individual, group,
community, populaiton, society that focus on motivation by nursing
action. The standards of practice applied in practical settings as
a caring, principle of primary care it need individual and family
partnership and empowerment, standards that improve development and
application of community care processing. as an important component
of community care assessment, planning, intervention and evaluation
important for fulfilling people's needs. partnership with the
individual, family by knowing, understanding the needs, knoedge
utilization, the evidence-based practice should focus on care.based
on community-level change model and theories, nightingale's
environmental model, banduras social learning theories, rogers and
theories make amultifaced intervention that should be implemented
for community-level care. many theories and models support health
promotion and disease prevention, it guide for prevention program
that focuses on health problem that should be addressed, served and
implemented. Example: In rogers theory focus on person-centered
realtionship that promote self-esteem, self-efficacy. Nightingale's
environmental theory for community care focuses on the action by
using the environment to assist patients and recovery. when a
patient is sick consider them as an individual with best interest,
concern, and trusting relationship that ensures they gain knowledge
and empower them to make decision.
