2. The measures to reduce adverse effects of medication administration are :-
- nurse should monitor the vital signs before and after administration of medication
- follow the 6 R's of drug administration
- Assess the renal and hepatic functions before administration of medication
- Check for any allergic reaction to the driver before administration
- Check the therapeutic levels of the drug in the blood and monitor for any signs of toxicity .
3. Use of the 6 R 's of drug administration which include :-
Right drug
Right patient
Right time
Right dose
Right route
Right documentation
And double checking the medicine and drug book with the help of a senior staff at duty . If the nurse has any confusion regarding the medication order clarify it by asking to the doctor on duty directly and if not available then telephonically.
To make understand the medication regimen to the patient , explain him all the medicines he is taking and explain their effects and side effects to help him to cope with the co.plication at an early stage itself . If any medicines are added explain that also to the patient and family . Include the timings and route of administration while explaining so that any miss of dose or wrong route of administration is avoided .
