Yes, I have requested the hard copies of my medical records when I
was diagnosed with tuberculosis. I wanted to have my diagnostics
done in a different laboratory and have a second opinion before
starting the medical treatment regimen. I had requested the medical
records department after showing them the hospital patient
registration card, prescription for the test and doctor's reference
and patient medical record number. The medical records staff did
not charge any money for the records and I revived the records
after a day. During the handing over of the records my identity was
confirmed by an identity proof, patient number, hospital
registration card and doctor's prescription.

Yes, i do maintain a hard copy of all my family member's medical records in a folder. This is a practice that my family maintains which helps us to have the records handy whenever required as the technology driven electronic records are not easily accessible by the family members as the older parents and children are not technology savy and prefer to have hard evidence of the health records. The system I keep to organize the records is to have all the allergies, frequently occurring medical conditions and associated medications on the ton with increasing order of the priority. This is a good tip as any doctor or healthcare person when reviewing the medical records can easily get the vital and critical information about the each family member on priority and not waste any precious time in case of medical emergency

I do use the electronic patient portal to maintain your medical
records as they are readily and easily accessible and very helpful
to share to the doctor in case of emergencies and treatments. As
the records are kept on the portal, it can be accessed anywhere at
any time and making copies of the same is a good help of
technology. As mentioned previously, my back-up system is to
download the files on my personal computer and smart phones in
order to be not dependent on the portal functioning, connectivity,
internet and service issues, etc. My organizational tip is to save
the records on your most easily accessible device like a smartphone
in a folder with a password locked condition. This helps for you
and only you to access the medical records while maintaining the
security and safety of the same

I once participated in a clinical trial study that studied the
effects of anxiety medication on day to day functioning and their
adverse events involved. I was concerned that the researchers might
publish the study result along with my name and other details and
this might let other people know that I am taking such medications
and affect my personal, professional and social life. So I
requested for my name, address and other relevant personal data to
be removed to the physician and the researcher. On interacting on
the request, they assured me that under HIPAA law my privacy is
protected and even if the data is published my name and sensitive
personal information will not be revealed at any time and that my
privacy ad safety is taken in serious consideration and would not
be revealed to the general public.
