Answer - Some evidence based for prevention against pressure ulcer is as follow -

- Always assess the skin and its integrity most frequently.

- Always monitor risk factors which develop pressure ulcer, try to avoid risk factors.

- Appropriate positioning, using pressure relief devices, proper meal in diet, proper fluid are most favourable for client.

- Turn and reposition during immobile patient every 2 hour, active and passive range of motion exercises help to prevent against pressure ulcer.

- Apply cream or lotion for skin lubrication.

- In your bed, bedsheet should be wrinkl free.

- Prevent from overweight, it cause more pressure on skin and develop chance of pressure ulcer due to more pressure.

- Drink a plenty of water everyday for maintain good hydration because water is good for skin.

- You should also monitor your cloths, if it should be wrinkl free or should be light. Avoid tight cloths.
