Diabetes: A group of metabolic diseases where by a person has high blood sugar due to an inability to produce or inability to metabolize sufficient quantities of the hormone insulin.
Hypertension: Abnormally high Blood pressure is called Hypertension.
II) a. Theory: Health promotion model by Nola J Pender ( 1982)
b. Pender's Health promotion model:
Nola. J . Pender is a nursing theorist born on 16 August 1941 at Michigan, US. She proposed the health promotion model in the year of 1982. This model describes the multidimensional nature of a person's health status and the behaviour that is focused on promoting health is the behavioural outcome. The basic assumptions of this theory are,
* Individuals can actively regulate their behaviour
* Individuals can interact with the environment by transforming the biophysical environment or by transforming self in terms of Environment.
* For changing a behaviour pattern, one have to initiate a self configuration and make changes in the environment.
* Health professionals, especially nurses, who are a part of interpersonal environment can influence the person's behaviour and health.
Pender's Health promotion model, emphasise that health care providers, families, peers can motivate/ influence health promoting behaviour , that can increase health status of the individual and will prevent the diseases or its complications.
c. Justification of choosing the Theory.
Nola J Pender's Health promotion model can be applied in any individual' s situation related to health. It focuses mainly on adapting the health promoting behaviour, so that individual can prevent the diseases or complications of already developed disease. In the above scenario, the hypothetical patient,is suffering with Diabetes and Hypertension. If she is not controlling the behaviours that is undesirable for Health such as eating restaurant foods , junk foods and Physical inactivity, she will develop many complications associated with Diabetes and Hypertension. According to Health promotion model, here Healthcare Provider / Nurses can influence the patient to change her behaviour and to adapt health promoting behaviours , to change her environment for preventing further complications.
d. Mrs. Juana Gracia is suffering with Diabetes and Hypertension. She is an obese lady with risk of developing heart disease or stroke, if she is not controlling her diet and life style . According to Pender's Health promotion model, nurse can interact with her and establish a therapeutic relationship, so that she can influence Mrs. Juan for adapting health promoting behaviours such as heart healthy diet with low calorie and low fat, regular exercise, weight loss etc. She can support her to avoid any drinking habits, if any. Motivate the patient to change her behaviour of consuming unhealthy diet, inactivity etc. Educate her the risk of not controlling Sugar level or Blood pressure, that can lead to Coronary heart disease, stroke, heart attacks, renal failure etc. If the patient is ready to accept the health promoting behaviour of healthy diet, weight loss, regular exercise, she can largely prevent the occurance of complications.
(F. Usually when ( I) apply Health promotion model to my own health and for my patient's situation, it proven its effectiveness . Those whose affect or attitude to change is positive, then it will be easy for adapting health promoting behaviour. For me , when My attitude towards lossing the extra weight become positive, I easily motivated to do exercises, avoided junk foods and artificial sweeteners etc. That helped to loss extra weight. Nowi can keep the idel body weight, not less not more.)
