1. Situation :

The patient-reported unnecessary repeating of blood tests. Occurred do the lack of continuity of care and miscommunication. The patient thinks that it was due to lack of communication.

To avoid this problem and ensure continuity of care, all practitioners involved should have complete, up-to-date, and accurate information about what other practitioners have done, especially about tests done and drugs prescribed.

2. Situation:

A stroke patient was discharged to the home setting the caregiver had made sure that the patients family members are provided with enough information to adequately care for the patient in that setting. Proper guidance about the care of the patient after stroke will help in early recovery. Information about follow up was also given.

3. The effective case management includes five components: assessment, planning, implementation, evaluation, and interaction.

A client admitted with the fracture of the femur is assessed for actual and associated symptoms, proper care is planned to alleviate the symptoms and aids recovery. The planned care is implemented effectively and timely evaluation is done and re-planning of care was done where appropriate. Interaction with the patient and family is very important in applying case management approach.

4. In case of fracture patient

Health teaching: Health teaching about nutrition, positioning, exercise, complications and follow up can prevent and reduce further injury. It can also promote healing.

Referral and follow up:

Referral is an essential aspect of care. When a patient needs additional support and care, a healthcare personnel should do it in time.

Follow up can be used to monitor health, reinforce knowledge and action plans, decide on modification of medication plan.

5. Care of patient needs the continuity of care as an essential component. The department policies should enforce the continuity of care by all personnel.

The charting guidelines used by a hospital should include the proper guidelines to ensure continuity of care.
