Continuity of care is very important to the patients for maintaining a better healthcare outcomes.Continuity of care is very important to older patients who discharge previously from the hospital.It will leads to the better satisfaction to the patients and to maintai a burden free health.

1.The main reason for lack of continuity of care is is lack of capacity and time that is increase work loads that is result in difficulty to access care to the patients who needs continuity of care.it is very important to follow the continuous care to the patient by avoid the difficulties.As a nurse in charge we have to ask the difficulties faced when they are coming for the follow up's.

As a nurse in charge it is very important to access the continuity of care.we have give important consideration to this problem and allot the staff for the area who need follow up's and continuity of care.We have allot particular time to the different staff who are in alloted in this area.Thus as nurse in charge we can manage tis problem very smoothly

2.To avoid the lack of continuity of care we have to plan regarding the continuity of care before the discharge of the patient. Before the discharge itself we can mention regarding the importance of continuity of care in the discharge planning.During the discharge process we have to explain or educate regarding the importance of continuity of care as well as where they have to come such as department and whom they have to apporoach etc.Thus we made the care effective when they come for the follow up's

3.Case management of a patient who is suffering with anemia

a.Assessment:(subjective and objective data)

  • The patient complaining of fatigue and tiredness
  • On assessment pallor of eyes,decreased capillary refilling,coated tongue,altered skin turgor

b.Planning

  • Collect the baseline data(including history,food habits and vitals)
  • Make the patient comfortable
  • Administer blood or start iv infusions as prescribed
  • Check the vitals and lab investigations and inform to the doctor
  • Educate the patient and family members regarding the importance of taking iron rich diet
  • Encourage the patient to drink plenty of oral fluids
  • Educate the patient regarding the importance of continuing care

c.Monitoring

  • Carefully recorded the base line data and informed to the physician
  • Adminstered 1 pint whole blood to the patient
  • Administered 1000 mlof normal saline as prescribed
  • checked the vital signs and lab investigations and informed to the concerned physician
  • Educated the patient regarding the importance of taking iron rich diet and oral fluids
  • Educated regarding the importance of follow up

d.The patient lab ivestigations and activity level improved

One of the old age patient is admitted in the ward due to age related medical conditions such as diabetes and high B.P and the is 85.While During the physical examination and contact with the patient there is bad odour from the body,bed sore,halitosis etc.The intervention strategies initiated in this case

a.Health teaching

  • Educate the family members regarding importance of maintaining personal hygiene to the dependent patient
  • This include Bathing of the patient,in this case the patient is bed ridden and we have to go for a bed bath and wiping of the parts,and make it dry
  • During dressing we have to get help from the assistance from others
  • Oral hygiene is really important particularly the elderly including brush and floss,wash and massage the gums with a soft cloth,clean dentures with soft brush if any
  • Special care is given to the bed sore such as clean,dressing if any and powdering of the sores and regularly change the position of the patient with support

b.Referral and follow-up

  • Many of the old age patients are dicharged with complex medical problems
  • During the discharge the discharge plan is summarized regarding the need of continuing care for older patients
  • Mention regarding the follow-up intervention thus the family members realizes the importance of continuing care

5.Increased patient volumes

Due to increased patient volumes most of the patients are not getting appointment to continue care.To avoid this barrier we have a systematic way to approach follow up.Thus we can avoid this barrier and increase the patient satisfaction and offera quality care.
