Mr K 40 year-old had laparotomy and removal of all bladder done five days ago.The doctor has decided to discharge him today.

You are required to provide health education on post hospitalization care for the above client.

HEALTH EDUCATION TO THE CLIENT----

1) you should obey the instructions of having the medication on time as given by doctor( like antibiotics, painkillers, stool softeners). Don't skip this medication and also try to finish your medication as instructed by doctor. Taking of pain killer must be on limit or daily permissible limit don't overdose .

2) Should take plenty of water . Diet restrictions should be there. Avoid oil and more fatty food. You may face constipation for this reason add more fiber( both soluble and insoluble fiber ) to your diet.

3) you should take rest as much as possible.

4) you shouldn't take bath or shower untill the doctor has told you if the incision area become wet it can infective. Avoid soapwater to the incision area. Keep the incision area clean and dry.

5) If you want to walk try don't fully give pressure on the body. It will be better if your family members helps you.

6) avoid doing exercise untill 6 week . Don't try to lift weight or any heavy exercise untill doctor tell you.

7) always try to keep your area coverd and dry. If you feel that the dressing upon the incision area need to be changed ( means if the dressing becomes wet) remove the dressing gently before that you should ensure that you have take hygienic and handwashing with soapwater.

8) If you see any drainage from the incision area try to observe the amount of drainage which is coming out and try to keep clean drain all time.

9) Don't rub oil, cream on the incision area.

10) use the splinting technique . When you sneeze , cough try to keep a pillow with you hand upon the area. It will decrease the force acted on belly.

11) you should visit to your doctor as the doctor instructed you for the followup.

12) If you feel any problem like fever, chest pain, pain in the bladder region, urine output reduced,nausea vomiting, severe pain from incision area,dark urine, constipation,, diarrhoea contact with your doctor as early possible.
