You are assisting in the restoration of a mesial-occlusal-distal (MOD) amalgam on tooth #13. How many wedges should be set out for this procedure? Where is a wedge positioned in preparation for proper contour? From what direction do you position the wedge? HTM

You are to prepare the universal retainer and matrix band for tooth #29. When assembling the band into the retainer with the diagonal slot facing you, in which direction should the band go-right, straight, or left?

Before you assemble the matrix band and retainer or place the Mylar strip interproximally, what needs to be done to the band to enhance its contouring abilities?

The dentist has overfilled tooth #13 and has completed the initial carving and is ready to start the final carving of the occlusal and interproximal surfaces. Describe the sequence involved in removal of the matrix band, retainer, and wedge

Solutions

Expert Solution

  • In the above case, two wedges are required for the restoration of a mesial- occlusal-distal ( MOD) amalgm on tooth #13. The wedge must be place intraproximally to avoid imbalance and to deliver a proper contour. The direction of placement of wedge should be in diagonal lost towards gingival surface.

Before you assemble the matrix band and retainer or place the mylar strip intraproximally, hold band securely in place by applying finger pressure on the occlusal surface while tightening the band securely on the tooth. Select the wedge at a right angle in the breaks of a hemostat or locking pliers. Then correctly place the wedge in the intraproximal space below the gingival margin of the tooth preparation from the ligual side.

Sequence of removal of the matrix band, retainer and wedge

  1. Collect appropriate armamentarium,PPE, safety glasses
  2. Remove wedges with cotton pliers
  3. Use explorer around band to loosen amalgm from band
  4. Remove matrix retainer while maintaining pressure on occlusal surface of the tooth
  5. Loosen the band by gently pulling horizontally away from the tooth while maintaining the pressure with a wall burnisher
  6. Remove band gently towards the occlusal to prevent fracturing the restoration
  7. Floss the tooth
  8. Massage the gingiva after removal of band
