Cleft lip and palate refers failure of fusion of fronto nasal process with the maxillary process.

Past medical history

Infant has no relevant major illnesses and disease.

Family medical history

Her family consists of her mother, father and elder brother age-4year. Family has no history of major illness.

Social history

She belongs to middle class family. Her father is earning member of his family. Her parents maintain good relationship with each other and neighbors.

Present medical history

Child has feeding problem due to ineffective sucking resulting undernutrition.

Child has planned for plato plasty. Mother is primary care giver. She look after her child very carefully.

Nursing diagnoses

  • Ineffective air way clearance related to aspiration of feeding as evidenced by dyspnea
  • Imbalanced nutrition less than body requirement related to inability to ingest food.
  • Parental anxiety related to inadequate knowledge regarding disease process
