Match the definitions with one of the following two terms: transcutaneous, transvenous.
Definitions: through the skin, through a vein.
Using the terms above, which type of pacemaker is implanted permanently? Which type of pacemaker is used temporarily such as in emergencies? List the indications for permanent pacemaker insertion. List a few arrhythmias that might need to have pacing in an emergency.
Transcutaneous means through the skin.
Transvenous means through a vein.
TYPE OF PACEMAKER PERMANENTLY IMPLANTED -- Transvenous pacemaker . it's permanent when transcutaneous pacemaker is not of use and this is done by electrode pacing through a central large vein to the node which is present near in right atrium or right ventricle.
TYPE OF PACEMAKER USED TEMPORARILY SUCH IN EMERGENCIES-- Transcutaneous pacemaker. It is also used in Symptomatic bradycardia along with in emergency like Myocardial infarction, complete heart block but it is temporary until the patient stabilize by permanent pacing . In this situation the electric current is being sent through the skin by placing it on the chest.
INDICATIONS FOR PERMANENT PACEMAKER INSERTION--
1) if atrium is involved in arrhythmia like -- most cases sinus node dysfunction.
2) Atrioventricular nodal block is present.
3)post cardiac transplant.
4) congenital heart disease.
5) neurocardiogenic syncope .
6) chronic bifascicular block.
7)In patient with severe systolic heart failure.
LIST OF ARRHYTHMIA NEED TO HAVE PACING IN AN EMERGENCY--
1) arrhythmia which involves the atrium like - atrial flutter, atrial fibrillation , junctional rhythm failure in diastolic period.
2) supraventricular Bradyarrhythmia like sinus node dysfunction.
3) ventricular tachycardia.
4) Atrioventricular nodal dysfunction like in second degress type 2 heart block.
5) Torsade de pointes.
