Informatics is a science with multiple information management and analytical sciences to identify, define, manage, and communicate data, information, knowledge, and wisdom in health care practice. It supports the health care team in decision making in all roles and settings to achieve desired outcomes.
An important area in improving patient outcome and making hospitals more efficient revolves around the challenge of patient readmissions.using patient data to identify historical numbers on readmission, informatics minimizes the gap between the improper health records transmitted due to human error. It is most useful for effective patient treatment which will minimise the chances of readmission. Nurses shift duties and improper communication can affect the health of the patient to greater extend, which is again minimized with the help of informatics
An study from yale school of medicine have determined that proper discharge summary can help to reduce the risk of readmissions. The study findings were that discharge summary quality was indeed associated with readmission risk, and that patient whoes summary included useful content or were send to outside clinic had lower admission rates. Online survey conducted by health catalyst stated that nearly nine out of ten nurses and health executives say their organisations are successfully improving the safety of patients, yet these medical profeaaprofes also identified ineffevinef information technology as key barrier for achieving their patients safety goals. Thus the above mentioned two evidences prove that informatics and readmissions go hand in hand.
Collaboratives can provide a way for health system and hospitals to team together and share best practice strategie for proeven ting hospital readmisaions for instance, new Jersey hospital association launched a year ling collaboration involving 50 hospitals, nursing homes and home health agencies to reduce hospital readmission. The goal of collaboration included, understanding why patients are readmitted, identifying best practice to reduce the rate, developing resoucere to improve care and to better manage patient condition, such collaboration is only possible with proper informatics.
Transitional care has also shown to reduce hospital readmission. It features a transitional care team or professionals who facilitates coordination and continuity of care for patients as they change providers post discharge. One study conducted at Baylor medical center at garland found a nurse-led transitional program reduced admission rate by 48%.
Installing telemonitorinng technology in homes of chronically Ill patients can also reduce the readmission. Telemonitoring technology is also an informatie which prevent readmission. Patients record keeping via computer help to better track treatment and patient progress. Through use of computers, the internet and various medical database, doctors can better learn how to treat patients more effectively. The data received by use of health informatics provide statistical information that can have a profound effect on how medicine is distributed, surgeries are performed and how healing is tracked.
