What are the main forces on hospitals that have resulted in a reduction in the total number
of hospital beds in the U.S. and How has employment in hospitals been influenced beyond
utilization of beds
With excess hospital capacity , a growng perception exist that th capacity of some hospitals now seems be constrained. Some cases has been seened that the supply of hospitals services tend to be slow and out of sync with changes in the demand for hospitals services ,Those hospitals that are repoting capacity problem are often teaching hospital , located near previously closed facilities or in population growth areas. thats Why dealing with capacity problems might best to focus on better matching individual hospital supply and demand adjustments., No one has expects a hospital to be completely full all the time , maintains the reserve or standby capacity to deal with unanticipated health needs., whereas some excess hospitals capacity is desirable because it provides leverage to third party payers in their rate negotiations with the hospitals ,
Sometimes strained hospital capacity have been inreasing , including the reports of patients beinng held in one hospital unit because they were no beds available in a unit more appropriate to their needs and of hospitals temporarily closinng their emergency department because they were unable to accept more patient , some times patient do not admit due to lack of medical / surgical floor beds or intensive care unit (ICU) beds
The basic premise was that because hospitals were uncertain about the demand for their services , they tried to provide capacity not only tro meet the expected number of people who would require care on a typical day but also to have sufficient standby or reserve capacity to meet a larger than the expected demand,.
If a hospitals belives that it is not have optimal capacity , it is likely to adjust its supply of services , Maintaining too much capacity can entails costs that may not be compensated by existing payment methods and thus may detract from a hospital's viability . too little capacity means that the hospital isturning away too many patient , The literature on supply adjustment indicates that these adjustments , wether they involve either increases or decreases , can be quite costly for a firm and thus are likely to be made slowly .
During the some drastic situation such as epidemic in a certain community area or the pandemic region , the emergency department of hospitals are over croweded. , , sometimes in the gynae department , there are more deliveries of the pregnant women , but the available of beds are limited , so, many hospitals department does two women to shared one bed , that really strained condition to be faced by the patient also,
The best suggestion is to be that strained capacity in various hospitals units led to have a backups in emergency department or any others over crowded department as like the gynae or medicine wards , . Increasingly this media and hospitals advisory groups to be raised the possibility that hospitals might need to expand rather than reduce their capacity ,
Hospital bed utilization is influenced by the various fcators , which may be categoirized into patient related , also physician related ands administration related issues, Inappropriate hospital bed utilization wpuld be percieved differently by the clinicians and the patients as well .,. Given the unique role of clinical practitioners in quality care , their views on the causes of inefficient hospital resources use should be reffered to any improvement initiative.
The ineffective hospital information system , absence of standard operating procedures , and lack of quality assurance system , in the ward management as potential contributing factors to overutilization of hospital resources .. Such as those related factors contributing to patient overstay may help the policy makers and administrators to promote more efficient utilization of healthcare system resources . By implementation of quality assurance system in the ward mangement , revising the admission and the discharge policy , improving hospital informayion system , use of modern technology , providing the clinicians with the appropriate training may help in alleviate the patiennt overstay and suboptimal utilization of resources in hospitals .
