Cases in health care management of the handoff process reflect on the hand-off process
Handoff or handover means the transfer of patient care from one health care provider to another or from one health care facility to another. These handoffs are important and plays vital role in the development of an institution. It is the weak communication system but the most important one also. The impact of giving poor handoff in an organization is very high. There are many types of handover in the healthcare setting. For eg, RNs to RN, doctor to doctor, other health care professional to another one . These helps in patient care by providing a continuity of care. In this handoff, the first staff gives full details of the patient, ie, what all things/ events happened in their shift to the next shift stafffor providing a continuous care. And it helps the avoidance of repetition of procedures. The transfer of essential information naf and the responsibility for care of patient to each other is an integral component of communication in health care.
Not only, the handoff in one hospital is important. Ie, inside the hospital. If the patient is referred to other hospital for further or better treatment. Then the staff or personnel cared the patient in first hospital have to provide full treatment history and hand off to the staff in the next hospital. Then only they can provide continuity of care.
As a patient moves from specialised services within a hospital as shifts of medical personnel come and go, there are numerous episodes in which control of, or responsibility for, the patient passes from one health professional to another and in which important information about the patient is also exchanged. While giving and taking hand off they can clarify their doubts by asking questions each other.
Poor handoffs impacts an organization's quality of care why because poor handoffs increase the frequency of mistakes. And this reduces the benchmarking of institute gradually. In view of the growing interest in improving hand off processes and the need for guidance in arriving at standardized handoff procedures. Although accurate and appropriate treatment of patient care information is extremely important.
Written handoffs are more effective then verbal handoff. Report writing and proper documentation will prevent legal issues and it makes the healthcare provider and the institution safe from the legal conflicts. In nursing, bedside handoffs are more effective ie, wikehand off time they can check the details explained by the staff in the same time. These handoffs improve communication between them and patient safety.
As a manager, we can ensure proper handoffs between the staff by maintaining a proper documentation of events occurred in each shift.it can be written or computerized. It should contain detailed manner of care. Bedside hand offs are more effective than handoffs away from the patient side. If they have any doubts they can rectify during the handoff time itself. This will reduce the wrong practices and repetition of actions.
