For patient care and good health care system we need improvement in record keeping and health IT..some of the elements of a health care system promoted by leaders still it is to be consider by the way to make change and crucial..
1,Reform will not succeed unless electronic records standardize on a common,robust format:
   In health care field patient record should be by data analysis applications to make proper diagnosis,public health planning and research into new treatment..consumers choosing a particular hospitals by data about hospita procedures,costs and patient satisfaction..data-wise physician's are suffered in 1980s for proper record system for the different department..Now vendors are taken responsible for both government and the market by promoting interoperability..federal governement taken responsible for this data..but this wont't be sufficient. every one must take responsible for this..
For exchanging patient data we need multi-million dollar investment..HLT commitee,provides standards for electronic health records,it deal with new,complicated histroy and formats it make compatibility among vendors..the direct project and RHEX RESTful exchange standard helps hospitals exchange for information with government forces exchange..But we need to extract useful data from records..
2,Reform will not succeed unless massive amounts of patient data are collected:
      we need the patient data to provide most effective treatment..but we can not prescribe drug for the patient without collecting enough data about patients and that drug reaction..we don't know how the drug will be safe for the patient..we are tracking their vitals and treatment plans for the risk population on a daily basis..some institutions follow holistic approach to health when doctors and hospitals following patient records they should not add anything more in their data..
3,Reform will not succeed unless patients are in charge of their records:
   doctors are providing their own decision care without sharing with patient their information..sometimes one drug can interfere with another it makes patient falls for their followup plans..any data collected by the patient must be in scribbled notes..sometimes doctors will not trust patient because medical record are complicated documents if there is one small error can make a life at risk..so it is better to bring the patients to team and educate them..
   Genomic information and radiological images can occupy dozens of gigabytes of space..most of the hospitals are following cloud .Among1,50,000 medical practitioners only one in four doctors selecting an EHR for practice fusion..Doctors can treat patients in asking their history,vital signs and observations of daily living..US governement stimulate patients empowerment by requiring doctors to open their records to patients..but some institutions providing patient log for them..it is important to navigate..
4,Reform will not succeed unless providers conform to practice guidelines:
Now government is forcing doctors to release information for patient to choose hospitals and doctors for thier choice.. medicine is a science and art..patient choice is restricted by insurance rules,it also make difficulty to determine the procodures cost..cost structure will benefit patients and tax payers..some insurers have started new payment mechanisms..
5,Reform will not succeed unless providers and patients can form partnership:
      when we talk about technologies and articles poor model education,data collection for patients will be lacking..patient want time to talk with their paroviders for their needs..data and good record keeping help for efficient resources and deal with the physician shortage can spread job among other staff's..Relationship is important for health IT to make efficient care..because of science empowerment personal connection have lost..Good records help doctors to know patient well..but communication is important...
