One of the challenges faced by the health care provider when providing continuity of care poor knowledge of the child on the part of the service providers.Only when a helth care provider understands the child better and create relationship ,a better health outcome coul be expected.Some of the other possible chalenges maybe institutional structures and practices,geographic factors,provider attitudes and parent preferences all may contribute to hinderance of continuity of care.
