Scenario: A 35-year-old man, former Army sergeant, was involved in a multi-vehicle accident on a major highway. He was relatively unhurt, but witnessed several bodies strewn across the road immediately after the accident. This caused him to experience “flashbacks” from an active Army combat zone several years earlier. He begins to have nightmares of the accident and becomes unable to function at home. His wife accompanies him to a Crisis Unit where he is subsequently admitted to the Mental Health Unit of a local hospital with a diagnosis of Post-Traumatic Stress Disorder. Initial Discussion Post: Address the following: What are the most important factors for the RN to assess when dealing with a client who is exhibiting flashbacks? How might the RN foster therapeutic communication with this client? List three common flashback triggers for former military personnel and describe how the RN can assist to decrease the effects. Provide two 3-part nursing diagnosis statements that might apply to former military personnel experiencing flashbacks. Each statement must include an actual NANDA-I nursing diagnosis, a related factor, and “as evidenced by.” (No risk-for diagnoses permitted.)

In general population 8%men and 20%women with traumatic event developed PTSD.. national Vietnam veteran readjustment study found that veterans are high in war zone with PTSD..more than 4,42,000 veterans receiving treatment for PTSD..as per national centers for PTSD website,including the primary care.. PTSD is often associated with TBI,MST,sleep problem,substance use,pain and other psychiatric disorder..

Treatment include Trauma-focusd psychotherapy,anxiety management,stress reduction,guided imagery,congestive behaviour therapy and social support..

Pharmacotherapy include selective serotonin re-update inhibitors or serotonin norepinephrine re-update inhibitors..

Polytrauma types of traumatic stressors related to war include serving in dangerous military role..in addition to the formal diagnostic process,PTSD patient would include a determination of comorbiditycosymptom serving,functional status,neuro psychological impairment and malingering..more than 80% of people with PTSD will have major depressive or another psychiatric disorder..

A diagnostic instrument as per NANDA such as clinician administed PTSD scale to see the impairment in social and occupational functioning..neuropsy neuropsychological testing might be used to validate with confusing,attention,working memory,PTSD psychiatric disorder include ad per NANDA generalized anxiety disorder,chronic pain syndrome,post concussive syndrome and substance abuse..some studies shows evidence of PTSD related impairment exists are attention and memory and executive functioning and global intellectual functioning..
