You are the primary care provider for Mrs. Z, a 70-year-old female patient who has suffered from chronic back pain for a number of years. You are well acquainted with this patient, and due to recent changes in prescription regulations you now see her once a month to renew her narcotic prescription. Three months ago Mrs. Z’s 17-year-old granddaughter moved in with her, after a fight with her mother. Z’s granddaughter was a straight-A student prior to a car accident at the age of 15. She dropped out of high school last year and spent two weeks in rehab for substance abuse. Mrs. Z states her back pain has been worse recently, and she seems to be running out of pills before the end of the month. She is requesting a greater number of pills be prescribed. What concerns do you have? What questions would you ask Mrs. Z? What approach would you take with her? How would you help this family?
In this case first of all i will check the dosing frequency of the Mrs. Z which she is using for the chronic back pain. Then i will check the dose and other related tests for the current assessment of the condition. Then the last prescription date and all, I will check.
In this case her granddaughter had a car accident at the age of 15. And she is dropped out of high school recently and had spent two weeks in rehab for substance abuse.
And she is moved with her grandmother i.e. Mrs. Z, after a fight with her mother. May be due to the drug abuse tendency she is urging Mrs. Z to request for a greater number of narcotic pills.
Then i will ask the Mrs. Z regarding the above issue and will ask for the reports of the increased back pain and related tests. Then will check the dependency of the pills and will ask for the drug abuse of her granddaughter and will counsel to refuge for the demand of increased number of pills if it is due to her granddaughter’s influence.
Then i will counsel about her career and all then make an emotional impact to justify the situations.
Then i will take the final action in this case of the family as the concerned person.
