A 66 year old man in undergoing a bronchoscopy in the ICU. He is given midazolam pre-procedure. During the procedure, the patients respiratory rate and oxygen saturation starts de-creasing. What action should the NP take?

Solutions

Expert Solution

Midazolam injection is used to produce a sedative effect, sleepiness or drowsiness and relieve anxiety before surgery or certain procedures. When midazolam is used before surgery, the patient will not remember some of the details about the procedure.

The major side effects of midazolam is respiratory depression. The incidence of such events is higher in patients undergoing procedures involving the airway without the protective effect of an endotracheal tube. But in surgical cases it is rare. In such cases,

  • Provide for continuous monitoring of respiratory and cardiac function.
  • Immediate availability of resuscitative drugs and age- and size-appropriate equipment for bag/valve/mask ventilation and intubation, and personnel trained in their use and skilled in airway management should be assured.
  • High flow oxygen administration.
  • Start ventialtory support for severe depression in respiratory rate and oxygen saturation.
  • Keep the availability of specified anaesthetic practitioner while on surgery.
