1) PRVC
PRVC is Pressure controlled ventilation with a volume control. These have the advantage of both pressure controlled and volume controlled.
Advantages
▪ pressure automatically get adjusted for changes.
▪ decelarate or reduce the inspiratory flow rate, which helps in improvwd gas exchange.
▪ each breath is analysed.
▪ limits volutrauma and prevent hypoventilation.
▪ guaranteed tidal volume with minimal peak inspiratory pressure.
▪ decrease work of breathing.
▪ minimize the risk of barotrauma due to increased pressure.
Disadvanatges
▪pressure delivery is depend on previous tidal volume
▪ not suitable for asthma and copd patients.
▪ may worson positive end expiratory pressure.
▪ when patient demand or respiratory rate increases , ventilatory support decreases.
▪ poorly tolerated on awake or non sedative patients
PRVC is applicable in:
▪ patientswho needs less pressure and consistent tidal volume.
▪ patients with chances of changing workload of breathing ,CL or raw.
▪ acute lung injuary and acute respiratory distress syndrome
▪ patient requiring high or variable minute volume.
▪ patient who cannot spontaneously breath.
2) non invasive ventilation refers to the mechanical ventilation to lungs without artificial invasive airway.
Advantages
▪Decrease the workload of breathing
▪avoid intubation complication
▪ affordable cost
▪ minimal sedation requirement ( if needed)
▪ prevention of airway collapse
▪ better tolerated
Disadvantages
▪Patient feels discomfort with mask
▪increased aspiration risk
▪ facial skin necrosis and eye irritation
▪ slower correction of gas exchange
▪ suctioning of secretions
Indication
▪Acute pulmonary edema
▪Pneumonia
▪COPD
▪Asthma
▪respiratory failure
▪Obstructive sleep apnea
▪Motor neuron disorder
