Mr. Simpson is a 56 yr. old African American from Mississippi, who now lives in New York. He has been a type two diabetic for the past 10 yrs. Recently, he was diagnosed with hypertension. He lives with his wife, who does the meal preparation and his daughter. His only exercise is walking to work (roundtrip 1 mile) and rarely brings lunch to work. He was recommended to visit a dietitian. He comes into your office with his wife and is body language tells you (arms and legs crossed) that he does not want to be there.
How do you address Mr. Simpsons supposed resistance?

QUESTION 1

First greet the patient. Say hello Mr Simpson. Then maintain good rapport with the client. Many patients are never disclose their problems if he or she feel uncomfortable.  Don't  argue or blame the patient. Allow them to speak their own problems. If patient reluctant to speak , emphasize the importance of visit. Here, from Mr Simpson's body language understand that he is nervous and not interest to visit dietition may be because he doesn't know the impotance of this meeting. In such circumstances, dietition explain the significance of this visit.

QUESTION 2

From Mr Simpson case history realized that he didn't follow healthy diet. Most of the time he didn't take their lunch at work place.

So i would suggest combination of diabetic as well as hypertensive diet.

  • Small and frequent diet is good for DM patient. It help to maintain glucose and insulin level and prevent hypo/hyperglycemia.
  • If possible,check their blood sugar level before and after food.
  • Encourage them to include more vegetables and fruits especially apple, avacado, berries and kiwifruits in their diet.
  • Emphasize the importance of having lunch.
  • Aviod junk food consumption.
  • Avoid carbohydrates, sodium, coffies and beverages in their diet.
