Lack of accountability of physician/ health care professional on quality care may decrease the patient's trust on physician and on healthcare . Accountability of heathcare professional on overall quality, cost of care and for decision about local capacity, must be increased in order to improve the value of Healthcare. )

* B :The wide spread belief that more Medical care means better medical care.

( Public also blinded to the fact that more in number is more in quality. It is important to create awareness among public that quality is not every time equal to quantity. Better or quality in medical care is not at all refering to more number of servives or more number of diagnostic tests.)

* D : A payment system that rewards volume, growth, intensity regardless of value.

( Vloume based payment system largely reduced the value of Healthcare, as many physicians took advantage of it and increased the number unnecessary procedures or diagnostic tests to patients. Now a days the trend is changing to Volume based to Value based care.)
