Case Management Process and implementing change (motivational interviewing, change process with precontemplation to maintenance).

Expert Solution

Answer: Care management process can be explained as the process through which case managers provide health and human services to clients/support systems. The process consists of several phases that are iterative, cyclical, and recursive rather than linear and that are applied until clients' needs and interests are met. There are four key components within this definition that make up successful case management: Intake, Needs Assessment, Service Planning, and Monitoring and Evaluation.

The five stages of changes are:

  • Precontemplation.
  • Contemplation.
  • Preparation.
  • Action.
  • Maintenance.

The Motivational Interviewing (MI) technique was developed as a result of the ambivalence displayed by individuals with substance use disorders to therapists during both the initial assessment of their issues and their treatment. People in a precontemplation stage have no intention of changing their behavior for the foreseeable future.
