As the HIM manager, you initially plan to visit www.ncqa.org and review professional literature to research HEDIS data element requirements.
1. Develop a list of the care measures tracked among different patient populations. Review National Committee for Quality Assurance (NCQA) report on “The State of Health Care Quality 2011: Continuous Improvement and the Expansion of Quality Measurement.”
2. Develop a summary of the patient populations reviewed and report findings relative to different conditions. Briefly summarize the chronic diseases measured, children and adolescent conditions, and older adult conditions from, the 2011 Quality Measurement Report.
1) National committee for quality assurances (NCQA) released their 2011 state health care quality report which examined quality data from more than 1,000 health plans that collectively cover 118 million amerricans this report consists of documented performance trends overtime, tracking variations in care and offers recommendations in quality improvement,The health care effectiveness data and information set(HEDIS) is a tool used by more than 90 percent of american health plan to measure performance on the important dimensions of care and service altogether HEDIS consists of 92 measures across 6 domians of care, because so many plans collect HEDIS data and because the measures are so specifically defined. The HEDIS care measures are 1) weight assessment and counseling for nutrition and physical activities for children and adolescents 2)childhood immunization status, 3)cervical care screening, 4) avoidances of antibiotics treatment in adults with acute bronchitis, 6) use of imaging studies for low back pain, 7) medcation managment for people with asthma, 8) controlling high blood pressure , 9) comprehensive diabetes care,10) annual monitoring for patients on persistent medications, 11)children and adolescent access to Pcps , 12) perinatal and postnatal care, 13) well child visit in the 3rd, 4th, 5th and 5th and 6th years of life 14) ambulatory care. 2)a) the patient populations reviewed and report finding to different conditions: it shows different populations have different illness it may be from pregnant and prenatnal and postnatal children and adolsecent and adults and oldage, all have they different illness and diferent medical care measures are taken and reaching to them in most comfortable and curable senses to the patient,b) the chronic disease measure , by the NCQAs specifications address stnadardized performances measures tha t cover dimensions of care for people with asthma, diabeted , chronic obstructive pulmonary disease, heart failure and ischemia vascular disease etc.c) children and adolscent conditions and older adult conditions : measurement of health care quality for childrena and adolscent serves the same purpose as that for adults accountability, qualityimporvement, and population health , however the development of quality measures for child and adolscent population has been slow to emerge from the private sector because enhances quality is unlikely to produce short term reductions in health care costs , in contrast to the results associated with quality improvement efforts for older adults, they are 7 standards measures for the children and adolscent enrolled in medicaid program and these are assess care for them who are enrolled in medicaid manged care plan these measures involves some of the most common chronic medical conditions, preventive services and acute care,
