Scenario 1: The patient was admitted to the hospital with right lower quadrant abdominal pain. Following an ultrasound of the abdomen, it was determined that the patient had an acute appendicitis. The patient was taken to surgery where an acute appendicitis with perforation was confirmed. Scenario 2 Patient was admitted to the hospital after having a seizure. The admitting diagnosis was to rule out epilepsy. After testing was performed, the cause of the seizure was not determined. The attending physician documented that the patient did not have epilepsy. Final diagnostic statement was seizure of unknown etiology. Scenario 3 The patient was admitted to the hospital with fever, cough, and shortness of breath. After study, the physician could not identify the exact cause of these symptoms but felt that most likely cause was pneumonia. The physician’s final diagnostic statement was possible viral pneumonia Scenario 4 Patient with osteoarthritis of the right knee was admitted for a total right knee replaced. Prior to surgery, the patient developed atrial flutter and the surgery was canceled until the patient could be evaluated by cardiology. Scenario 5 Patient was admitted into the outpatient surgery unit for a cholecystectomy due to chronic cholecystitis. Following surgery, the patient developed severe chest pain and was changed to an inpatient. A cardiac catheterization was performed and identified extensive CAD. The physician’s final diagnostic statement was Chronic Cholecystitis and Arteriosclerotic Coronary Artery Disease with Unstable Angina
