The community of Finnytown has identified the need for a shelter to serve homeless women and children. Finnytown currently has a homeless shelter for men. Women and children can obtain health care services there but are not allowed to stay overnight. The Finnytown health care task force performed a community assessment that revealed that a higher number of homeless men than women reside in Finnytown, but the percentage of homeless women is steadily increasing. Results further showed that more women with children than men are living in poverty. The task force speculated that many women who are living in poverty are being overlooked and thus are becoming women without homes.

The task force and the community of Finnytown decide to open a homeless shelter for women and children. The new shelter will primarily serve women with children who are homeless or in poverty. Georgia B. is the community health nurse who is a member of the task force team. Nurse Georgia and other health care professionals are charged with planning health care services for women with children to be provided at the new homeless shelter.

Case Study:

Adam is a 6-year-old boy who dove headfirst in the shallow end of a swimming pool. Unable to move, he sank to the bottom of the pool and was rescued by his mother. His father called the emergency medical services (EMS) while the mother provided mouth-to-mouth resuscitation. The EMS arrived and continued the resuscitation and placed him in a cervical collar. Adam was placed on a backboard and began to breathe on his own while en route to a trauma center. The neurologic assessment revealed paralysis of the upper and lower extremities, and cervical spine x-ray images and a magnetic resonance image (MRI) confirmed a C6-C7 complete transection injury. The patient was intubated before going into surgery for decompression of the injury.

She tries to eat a healthy diet and uses supplements to give her added protection against chronic diseases, especially heart disease, which runs in her family. Currently, she takes ginkgo biloba to prevent memory loss, garlic to prevent heart disease, and fish oil supplements to keep her blood thin. My phone at home kept ringing and I could tell it was a FAX. It was...

My phone at home kept ringing and I could tell it was a FAX. It was from a hospital in our area. My fax on my printer answered it automatically. It was a patient's physical exam results being faxed to someone, but it didn't say "who" it should be going to on the cover sheet. I felt the hospital and employee were actually lucky it came to me rather than to someone who would violate this patient's privacy. I sent the FAX back with a "professional" note with it. I titled it HIPAA Violation in order to get their attention.  

I informed them I would mail the original back. I thought about shredding it, but felt it would be a good learning experience as to why you should CHECK a fax number before hitting send on the Fax machine. Same holds true for emails, right?

About an hour after I faxed back the document, I received a phone call from the Compliance Officer of the hospital. They informed me about the processes and actions they were taking. I was pleased that they called me and talked about what processes they were going to follow through with. They had already called the patient to inform them of the error and breach of HIPAA, etc. They were in the process of talking with the employee and discussing disciplinary action. They were also going to hold inservices with all employees to review privacy practices.

The processes they discussed with me are right in tune with what we teach/discuss in this class, and what you will learn to do when you enter the healthcare field. Think about the rules of faxing and HIPAA.
