Clinical handover or Patient handover is the transfer of professional responsibility and accountability of some or all aspects of care for a patient, or groups of patients,to another person or professional group on a temporary or permanent basis.

Case meetings is an opportunity to talk about the wellbeing of the patient in care and discuss any support needed, about any issues or acheivements.

Recording and Reporting Requirements for comprehensive Clinicsl handover are as follows

  • The first requirement of clinical handover is to ensure accurate and timely transfer of information, responsibility and accountability
  • Patient and carer involvement
  • Handover requires preparation
  • Handover records and reports needs to be well organised.
  • All written reports should be legible, with date and time updation of all care that is reported.
