For the scenario below, discuss the developmental, age, socioeconomic and transcultural considerations that should be considered when gathering the subjective and objective data and nursing care. Also, relate expected findings to those that are described and discuss any additional information that might be needed before a nursing diagnosis or care plan can be made. It is December in Maine. S.A. has brought her two children, ages 3 and 5, to the health clinic for the first time. She states that she recently immigrated from India. She is wearing a sari and a light jacket. Each of the children is wearing a light coat. Interactions between parent and children are appropriate. The children both fall at the 4th percentile for both height and weight. Overall nutritional status appears to be satisfactory.

Developmental, age , transcultural, soci- economic considerations:

a) Developmental and age considerations:

For the younger child as he is just finished his toddler period, nurse has to assess the achivement of developmental mile stones: physical and cognitive development. For both children inspect the overall appearance , check whether the growth and development is appropriate for the age, head circumference and chest circumference. Hearing and visual acuity also can be checked for the children.

b) transcultural consideration: There may be considerable difference between Indian and American children. Even in the standard birth weight there is a difference. Standard birth weight in India is 2.8, but in US , it is 3 to 3.5. Indian children especially in early childhood, have the experience with extended families and also their socialization may be more as they are getting pampered by more relatives. But they may not be more kind or soft. American kids usually are kind and soft in interacting  with others. Dietary restrictions may be there in certain religions , that may affect the child's nutritional status.

As S.A and children are newly migrated to US, the children may not be cooperative with others and health care staff initially , as everything and everyone is new to them.

c) Socio- economic Considerations:

Nurse can assess the socio- economic status of the family. As the kids should get good nutrition and education , parents should have adequate financial income or resources. Nurse also can collect the information regarding the rearing practices of children.

Expected findings can be relate with the development, age , socioeconomic and transcultural considerations. 4 th percentile in the Growth chart for a 3 and 5 year old Indian children is satisfactory and healthy, but should take necessary interventions for the improvement of BMI of the children according to the change in environment.

Additional informations:

* Check the family medical history and for any hereditary diseases

* Assess the history of any allergies

* Assess the prenatal, natal and postnatal history.

* Assess the childhood infections, if any

* Any specific dietary  restrictions according to the religious and cultural beliefs.
