A "Logic ModelTemplate" is otherwise called as a "Planning and Evaluation Tool" is a simplified model that can use to plan evaluate the program goals and objectives.There are some components in this model such as:Input or resources,Activities,Outputs,Short term outcomes,Intermediate outcomes,long term outcomes.This model we can say sometimes called as a "Theory of Change",why because at last in the outcomes due to the effect of resources,activities and output we will acheive an outcome ie,bring about a change.

The main concept of this model is the "Theory of change".It icludes the role the stakeholders also,why because they are having a main role in the health care organization.The main part of finance or funding of the organization is from the investment of stake holders.

To prevent or manage hospital acquired infection and to avoid finance burden for the organization(health care budget) we can apply the Logic Template Model.
