Patient health record system is a electronic record of the health status of a patient which also includes information filled by clinicians about the cares provided ,cost of service, insurance claims , investigation's done and their report etc. Patient can access it online using there laptops,tabs ,phone by log in the web site .

- limitations to data entry , validation , and information display.
