What are some roles that Case workers have?
CASE WORKER: A case worker is a professional who have degree in social work.
A case worker is a social worker who is emplyed by a government agency, non profit organization or another group to help the individuals with advocacy, information and solution.
Roles of a case workers in health care:
1) PSYCHOSOCIAL ASSESSMENT - case worker does the initial assessment of a patient' s present, relevant past and possible future modes of adaptation to both the normal and stressful situations, and their ability for problem solving.
2) Financial Assistance - Refer and assist the patients in getting financial assistance and health care coverage through government, and private programs.
3) Care planning - Assist patients and families in finding and arranging services such as hospital care, home care , community care etc.
Work with medical and nursing team and discuss about care planning.
4) Counseling - Advice and counsel patients and their families, explain the nature of illness, and how to effectively deal with it.
Serves as a grief counselor to help the patient and family to deal with trauma of illness and injury.
5) Give information about the policies, procedures and services both in the hospital and after discharge.
6) Provide emotional support during times of crisis.
7) Work closely with medical team, family and community stakeholders to ensure each patient has the support system which they need to recover from their illness and injury ( working with insurance companies to fund home health care service and out patient treatments).
