Discuss treatments associated with coartation of the aorta and indicate the incidence of hypertension following surgical repair.
Coarctation is a congenital heart disease in which narrowing or pinching in the aorta, usually between the aortic arch and descending aorta.There are two types of coarctation: infantile/preductal ,there is contraction between subclavian artery and ductus arteriosus.The postductal type constriction at the or distal to the ductus arteriosus.
Treatment:
Coarctectomy: Surgical repair consist of removing narrowed portion of aorta with anastomosis of the ends. In some cases , a graft of transplanted aorta is inserted where there is narrowing is so extensive that removal of anastomosis is not applicable. Surgery is mainly done of age 3 to 6 years.
Prevalence of mesenteric arteritis hypertension are high postoperatively.Hypertension commonly occurs during the first week after surgical repair of coarctation of the aorta.' - Such paradoxical hypertension usually resolves after several days provided that the repair has been satisfactory, but it frequently requires intensive antihypertensive therapy during the early postoperative period. One of the factors that has been shown to contribute to the subsequent risk of hypertension is the age at the time of the original repair.So repair is done at early stage to reduce complication.
