Nursing process comprises of autonomous and collaborative care of individuals of all ages, families, group and communities in all settings. We can define the Nursing process as the scientific, clinical reasoning approach to patient care that includes assessment, analysis, planning and implementation and evaluation. It is the most important tool for putting nursing knowledge into practice,and it is a systematic problem solving method for determining the health care needs of an healthy or unhealthy individual . It also provides knowlwdge on personalized care. Now we can discuss the following 5 steps:

Assessment:

It is the first step in the nursing process and involves critical thinking skils and data collection both subjective and objective.

Diagnosis:

The formulation of nursing diagnosis includes employing clinical judgement which helps in the planning and implementation of the patient care.

Planning:

This is the stage where goals and outcomes are formulated that directly have an impact on patient care. Thses patient specific goals and the achievement of such goals assist in ensuring a positive outcomes.

Implementation:

It is step which involves action and actual carrying out of nursing interventions outlines in the pan of care. This stage requires nursing intervention.

Evaluation:

This Is the final step of the nursing process and is very important to a positive outcome for a patient.Whenever a healthcare provider intervenes or implements care, they must reassess or evaluate to ensure the outcomes have been met.

How nursing has changed over times:

In early days of nursing, women learned medical skills from their mothers or other women in the same profession, Women were caretaker, so nursing was just an extension of what the roles at homes were anyways. Today the nursing profession has changed drastically over time. There are extensive training programs, more diversified staff and a level of prestige associated with the medical field that was not there before.

Time has changed has done a lot for many career paths but the nursing field has seen drastic changes to help the efficiency of medical care. Now, the process includes training programs, better hospitals, more responsibility and a sense of focus on the patient care which in turn has saved lives and created generation s that are dedicated to medical professionals.

Nursing in today's time includes the following:

Training- Earlier training was not even necessary and organized. Many sick was cared for by mothers and family members. Today, the qualifications for nurses are very specific and in-depth. There is a wide range of nursing programs, speciality, degrees and certification.

Setting- The health care setting used to be in-home or on the battlefield for many women in the nursing field. Now the medical settings for nurses are hospitals, physician’s clinic and health care homes.

Responsibilities- The responsibilities used to read a lot like a household chore list, in the early century. But changes in the responsibility have been introduced as comprehensive training, changing views of women and the need for medical professionals growing rapidly.

Culture- Nursing in the early 20^th century was known as being mostly female with a rudimentary amount of medical knowledge. Today, the culture has changed a lot, nursing is about education and health care knowledge and has diversified the gender norma and nursing salaries are rising.

Patient care- It is now an extremely important factor in the medical field for all medical professionals. The advancements in technology have created an environment that makes patient care more effective and helpful for the patient.

Nursing process, during treating a patient with a Foley Catheter:

Urinary Catheterization is the introduction of a catheter through the urethra into the bladder for the purpose of withdrawing urine.

The following process is carried out:

Preparation of the patient.

Retention or indwelling catheter or foley.

Procedure for insersion.

Caring for the patient with an indwelling catheter.

Removing the indwelling catheter and after acre of the patient.

The following are the steps;

1. Wash your hands with soaps and warm water for at least 20 seconds.

2. Empty the urine from the drainage bag into the toilet.

3. Place the clean cloth or gauze under the connector to catch leakage.

4. Pinch off the catheter with the fingers and disconnect the used bag.

5. Wipe the end of the catheter with an alcohol pad.

Nursing process, for back and neck pain:

At the beginning of an initial doctor’s visit for back and neck pain, a nurse is likely to conduct a pre-examination to prepare the doctor for the exam. This process includes :

Collection of information about current symptoms.

The duration of the symptoms.

How and when the symptoms arose.

Information on blood pressure, height, weight, current medications and other relevant health information are collected.

After the above collection of information, physical exams are conducted, such as palpation, strength testing, sensation testing, reflex tests and movement tests.

Further diagnostic tests are conducted if needed such as X-ray, CT scans, MRI, Injection tests.

Lastly, medication and required physical exersizes are introduced.
