5 elements of nursing process are: Assessment, Diagnosis,Planning, Implememtation and evaluation.

All this elements are done for providing care and ensure the safety of patients.

Assessment is the process of collecting data about the patients condition. During assessment we collect the entire details such as present and past medical history, family history, sociocenomic data, physical examination etc. The accurate data collection or assessment will help to identify the triggers present in the patients and proiblem of patient. This will help us to provide appropriate care to the patient.

Diagnosis is the process in which we are identifying the patients problem. After assessment we draw possible medical problems of patient or risks. And then arrange those problems according to the priority. This will help as to provide necessary care to patient. Diffrent patients had diffrent problems and varing in theie priorities. So proper assessment will Help us to draw correct nursing diagnosis according to its priorities.

In planning we are plan nursing interventions for each nursing diagnosis. The planning is based on the condition of patient and is to provide appropriate care. In planning phase we assume all the possible nursing interventions for patient according to the patient condition.

Implemetation of the planned interventions is the next phase. Here we implement the interventions according to patient condition. We should always think about the patient safety and comfort. Then according to this implement each interventions for patient care.

Evaluation is the last element of nursing process. In evaluation we are assessing the effect of nursing interventions and care provided to the patient. Due to the nursing interventions patient condition is improved or not is checked. If the patients condition is not improved plan new interventions for the patient.
