Dexamethasone is a corticosteroid (synthetic glcocorticoid) formerly used for anti-inflammatory and antialgic effects. It is tested on patients with COVID-19 in United Kindom's clinical trails and found to have benefits for patients in critical conditions. On treating the patients on ventilators, the mortality is cut off to one-third and by one-fifth in patients requiring oxygen. In Covid-19 patients, Dexamethasone dampens the immune response, when it becomes too aggressive.
Synthesis:
The first
-substituted cortical steroid derivatives were synthesized by Taub
in 1958, and obtained 16
-methylprednisolone
acetate which was considered as a potent steroidal
anti-inflammatory agent. To synthesize Dexamethasone, 16
-methylprednisolone
acetate is dehydrated to 9,11 dehydro-dervative. It is then reacted
with N-bromosuccinimide, a source of hypobromite, to form
9
-bromo,
11
-hydrin
derivative which determines the epoxide ring closure. The formation
of dexamethasone is made secondary to the ring opening reaction
with hydrogen flouride in tetrahydrofuran.
