The film's open ending can be interpreted in many ways. In my opinion, there are two points to be noted : firstly, the vampire has a pattern of killing "bad" men, the people who have contributed to making the place more dark and secondly, she has romantic feelings towards Arash. Even though she hurts many people, even until the end of the movie she does not harm Arash in any way and from the scene where she takes the earrings which he gives her and confides in him that she has done many bad things, we can observe that there is a bond of trust between them. She resists when she is around him, which means that she is consciously making a decision to not cause any harm yet and trusts him enough to gather all her things and get going with him.
The epiphany can be interpreted as Arash coming to terms with the fact that she has something to do with her father's death or it can also be that is completely exhausted and agonized by all the incidents happening around him and needs some air. He may have realised that there is a pattern to her killings because she supposedly kills bad men but sadly enough, looking at the things that Arash has done, he too falls in the category of bad men. Therfore there is harm to his life as well but there is not much he can do at the moment. The thought that his life is in danger flashes in front of his eyes and he contemplates all the ways in which he can save himself. He comes to a decision that he cannot just abandon the car and the girl and run away therefore he decides to go a little longer and then make his move, and so he gets back in the car and tells her that he has been affected by his father's death and so he needed a moment to which she nods.
If the film were to continue, Arash and the girl are now on their way in the car and it is quite an intense car ride and he is constantly plotting ways of killing her in his mind. He is also confronted by doubt when he realises that she may actually be in love with him and it is wrong to put an end to her and at the same time he is worried about what may happen to him if he continues to be with her. He is in a dilemma but finally decides that it would be better if he killed her. They make a pit stop and he mixes the drug that he had stolen, in her drink, as a result of which she becomes intoxicated. He then drives to a deserted area, kicks her out of the car and drives over her and does not look back, and the movie ends.
