Access-to-care policies are aimed primarily at providers and payment mechanisms with the purpose of expanding care to the neediest and underserved populations, including the elderly, minorities, rural residents, those of low income, and persons with AIDS.

No aspect of healthcare policy has received more attention during the past 20 years that efforts to contain increases in healthcare costs. Two major policy initiatives enacted by the federal government have targeted first hospital services (DRG and PPS) and then physicians' services (RBRVS) for price control.

Quality of care becomes increasingly important in the face of managed care and concerns about the implications of doing less. Funding to evaluate new treatment methods and diagnostic tools are increasing dramatically. Funding for research to measure the outcome of medical interventions has also increased. It is critical to evaluate quality in the light of both cost and access.
