Infant assessment, you note crepitus on the left clavicle. Additionally, the infant is not moving the left arm as much as the right. The Moro is decreased on the left side, and you are unable to illicit the grasp on the left hand.

Based on this assessment. What type of complication are you suspecting?

Infant is presented with Crepitus in left shoulder most common diagnosis is clavicle fracture.Clavicle is the most common bone injured during birth. Occurs as a result of a difficult delivery or trauma at birth. Factors increasing chance of trauma is due tothe newborn being large in size, the newborn’s shoulder getting stuck during delivery, a narrow birth canal, or the use of tools to assist with the delivery.

Infant having decreased movement of left hand and decreased Moro and grasp in left hand. So we should consider complications. Important complication associated with fracture of the clavicle during birth is the inability to move the arm due to an injury to the brachial plexus, or collection of nerves of the arm. The brachial plexus lies very close to the clavicle and may be damaged by the bone when the fracture occurs.  

so the infant should be screen for brachial plexus injury.

Following test used for screening injury

  1. Electromyography (EMG)
  2. Nerve conduction studies.
  3. Magnetic resonance imaging (MRI)
  4. Computerized tomography (CT) myelography.
