Social determinants of health are the conditions in which the people are born,grow,live,work and age .
They mainly influence on the money,power and resources at global ,national and local levels.
They are mainly responsible for health inequities between countries

They contribute to the development of health by poverty,unequal access to health care,lack of education,stigma,racism which leads to the development of illnesses among people
The communicable disease chain should have a Reservoir or a host (Human,Animal and environmental reservoirs) through a portal of exit,is conveyed by some mode of transmission(Direct_Contact ,droplet infection,Indirect _ Airborne,Vehicleborne,vector borne) and enters through an appropriate portal of entry to infect a susceptible host,hence called the chain of infection
The nurse can break the link in her practice by simple hand wash after each and every contact with the patient in hospital,whereas in community along with hand washing health education to be given describing the mode of transmission
