Ventilation refers to process of acquiring oxygen rich air from environment to alveolar sac or in other words it is inspiration.

Perfusion refers to blood flow to alveolar sac. Adequate blood flow is required for sufficient diffusion of gases or oxygenation of blood.

  • Ventilation perfusion mismatch occurs when there is either deficient ventilation or deficient alveolar perfusion. In case where ventilation is adequate but perfusion is deficient, then there will be less oxygenation of blood. When perfusion is adequate but ventilation is deficient then also there will be less oxygenation. This is called ventilation perfusion mismatch.
