INTRODUCTION:

?Mr.x is 31 years old male wit mild learning disability and boarderline personality disorder. He is from tamilnaTam , he was first assessed for social care service in tamilnaTam.so he is admitted in the local hospital and receiving intensive treatment in the protective environment.

HISTORY:

Mr.X parents divorced when he was 5 years old, he has had very little contact with them since then he is eldest of the three siblings. the childhood was characterized by upheaval and deprivation so that Mr X is difficulty to maintain relationships with their family.

PHYSICAL EXAMINATION:

Mr X is having the thoughts of self harm and making injuries to himself and physical disability is present.

CARE:

• Assess the day time activities supported , employment,or residential.
• Provision of specialist nursing services.
• Support for young adult transiting from children service to adult service.

TREATMENT:

MR x is hav learning disability and boarderline personality disorder for that psychiatrist need to give anti- psychotic drugs.
