Subjective data
Patient is complaining regarging warmth in the legs.
Objective data
By doing physical examination i came to know client having warmth in the leg and less pulsation.
DIAGNOSIS
Ineffective tissue perfusion related to increased coagulability of blood as evidenced by increased leg warmth and tendeeness.
PLANNING
Client will maintain optimal periferal tissue perfusion as evidenced by reduction in tenderness and warmth.
INTERVENTION
# Assess the general condition of the client like history or varicosities, immobility and smoking.
# Assess signs and symptoms for deep vein thrombosis.
# Monitor the results of diagnostic tests include duplex ultrasound, D- dimer assay, coagulation profile.
# Maintain adequate hydration and elevate the affected area.
#Provide below knee stockings as prescribed.
# Administer analgesics and antibiotics as prescribed.
#Administer anticoagulants as prescribed.
EVALUATION
Client can maintain optimal tissue perfusion with out clotting or altered immunity.
C) # Provide health education to the client who is having chronic vascular diseases
# Engage them with different kinds of activities.
# Provide self motivation and emotional support to the client.
