Answer 5: In nursing, assessment is the gathering of information on body physiology, and psychology of patients. This is the first step of nursing. Both physical and mental health is evaluated. These assessments are important to know the current condition of the patients and future care.
The physical assessments are pain symptoms, irregular body functions and any organ partial or complete failure. Mental assessment involves the behaviour, social engagement and emotions. The assessment is difficult for the children the subject, as they are unable to describe and hence direct medical techniques are used.
Other type of assessment is for studying the family history. Whether there was any similar disease in father, mother or their grandparents. This assessment helps in the knowing whether the disease in the patient is genetic.
Answer 6: The most common formal assesment is the study of family pedegree. In this technique, the blood samples of the family members are collected and a pedigree chart is constructed to the know the inheritance of the genetic diesease. Further molecular analysis helps to usnderstand the diesease mechanism.
