PHASES OF THE HELPING RELATIONSHIP
Pre-interaction phase
Before meeting a patient
- Review patient history including personal and medical
history
- Talk to care-givers and family memebers to gain more
information regarding patient
- Anticipate health concerns or problems that arise
- Identify a location and setting that fosters comfortable and
private interaction
- Plan enough time for the intial interaction
Orientation Phase
- When the nurse and the patient meet and get to know each
other
- Set the tone of the relationship by adopting a warm, empathetic
and caring manner
- Recognize initial relationship is often superficial, uncertain
and tentative
- Expect the patient to test ypur competence and commitment
- Closely observe the patient and expect to be closely observed
by the patient
- Begin to make inferences and judgements about patient messages
and behaviour
- Assess the patient's health stataus
- Prioritize patient goals and identify his/her problems
- Clarify patient's and your roles
- Form contracts with patients and clarify who will do what
- Let the patient know when to expect the relationship to be
terminated
Working Phase
- When the nurse and patient work together to solve problems and
acheive goals
- Encourage and help the patient express feelings about his or
her health
- Encourage and help the patient for self-exploration
- Provide information needed to change behaviour
- Encourage and help patients to set goal
- Take action to meet the goals
- Use therapeutic communication skills to facillitate successful
interaction
- Use appropriate self-disclosure and confrontation
Termination Phase
- During the ending of relationship
- Remind the patient that termination is near
- Evaluate goal acheivement with the patient
- Reminisce about the relationship with the patient
- Separate from patient by relinquishing responsbility for his or
her care
- Acheive a smooth transition for the patient to other caregivers
as needed
