You are a Nurse Director in a hospital located in an impoverished low-socioeconomic area of a large city. Recently, you have received a three-year grant to establish a Diabetic Health Center, affiliated to the hospital and serving the community around the hospital. The project will be evaluated at the end of the grant to determine whether continued funding is warranted. As a project director, you have the funding to choose and hire three healthcare workers, 2 nurses and 1 diabetic nurse educator, who are newly graduated and have inspiration to innovate and do something different, but still they need your help and support. You will essentially be able to manage the clinic as you see fit. The average age of your patients will be 55 to 70 years, and many come from single-live homes. In addition, the population with which you will be working has high unemployment and high crime levels, and great suspicion and mistrust of authority figures. You are aware that many restraining forces exist that will challenge you, but you feel strongly committed to the cause. You believe that the exorbitantly high diabetic complications rate and diabetic mortality can be reduced.

Impoverished socio ecnomic status within a age group of 55-70 years is a challenging factor in this situation.Single live home old age group will have a lot of social,ecnomical and Mental issues to deal.unemployment,crimes,substance abuse and non cooperation with authoriy will be a challenging refraining factors of this area
Driving Forces;
Diabetes mellitus is increasing its share of burden to the health-related problems in developing countries. Lack of effective screening for primary prevention has been one of the reasons for the rising burden.the risk of developing diabetes increases with increasing age.impoverished low-socioeconomic area lack health care benefits available in urban area due to a variety of constraints including the low socioeconomic status and lack of education. The results of this study have provided necessary inputs to categorize individuals with risk factors and to assess the risk of developing diabetes mellitus using a simple, safe, and inexpensive tool.
This project will be enhances targeted interventions to minimize modifiable risk factors and hence combat the problem of increasing diabetes-related mortality and morbidity in the economically disadvantaged group of the society.

The Project Manger uses the hiring authority to hire 2 Nurse and 1 Diabetic Educator.Project Manager can utilize there skills for Assesment,screening and educating the people.
Refreezing of the plan will affect the Diabetic Health Centre driving factors,This will lead to restructure the implementation strategies again after two years.
